We know that your home and your community are important to you. VillageCareMAX is a Medicaid Managed Long-Term Care (MLTC) Plan that was created to help you to continue to stay at home, and in your community, for as long as possible.

 

VillageCareMAX will work closely with your doctors, help set up appointments, arrange for your care, monitor your medications and guide you and your family toward the best quality health care options available.

 

VillageCareMAX coordinates health care services for chronically ill adults who wish to remain in their own home and communities for as long as possible.  The plan covers long term care services including: 

 

- Adult Day Health Care - Social and environmental supports
- Adult Social Day Care - Dentistry
- Audiology - Durable Medical Equipment and
  Medical/Surgical Supplies
- Home Health Care
- Rehabilitation therapies - Non-Emergency Transportation
- Care in a skilled nursing facility - Optometry
- Personal Emergency Response System - Podiatry
- Home delivered meals - Prosthetics and Orthotics

 

As a member of VillageCareMAX, you are assigned a dedicated Care Team – which includes a Registered Nurse who is responsible for creating and coordinating the plan of care that’s right for you.  Your care manager will visit you in your home to keep the Plan up-to-date about your medical and health issues, and to answer your questions.  The care manager coordinates all aspects of your care.

 

Working together with your care team, you will be able to live the life you want.
