VillageCareMAX is a part of VillageCare, not-for-profit organization whose mission is to promote healing, better health and well-being to the fullest extent possible.
For more than 35 years, VillageCare has provided quality health care to older adults and to those with other chronic diseases and conditions, including HIV/AIDS, who are in need of continuing care and rehabilitation services.
The VillageCareMAX Medicaid Managed Long-Term Care (MLTC) Plan was created to help you to continue to stay at home, and in your community, for as long as possible.
We work closely with your doctors, help set up appointments, arrange for your care, monitor your medications and guide you and your family toward the best quality health care options available. You will receive all of your Medicaid long-term care services through us.

The VillageCareMAX Full Advantage FIDA Plan (Medicare Medicaid Plan) is a new integrated managed care plan that focuses on providing higher quality care to individuals with Medicare and Medicaid.
We make it easier for you to obtain your Medicare and Medicaid benefits from one plan. Participants enrolled in the plan have no co-pays, deductibles or monthly premiums for covered items and services, including prescription drugs.
Your Care Manager will work with you, your family and a team of providers to help determine what services will best meet your needs. A Registered Nurse will complete a comprehensive assessment at your home when you first join our plan. Your dedicated care team with your help will develop a personal care plan with your needs – the Person Centered Service Plan (PCSP).
We will communicate with you on a regular basis and visit you as needed to ensure that you are getting all the services you need.
In addition to your medical, long term care services, supports, supplies and prescription drug coverage you will receive benefits such as:

The State of New York has created a participant ombudsman program called the Independent Consumer Advocacy Network (ICAN) to provide Participants free, confidential assistance on any services offered by VillageCareMAX Full Advantage FIDA Plan. VillageCareMAX Full Advantage FIDA Plan is a managed care plan that contracts with both Medicare and the New York State Department of Health (Medicaid) to provide benefits of both programs to Participants through the Fully Integrated Duals Advantage (FIDA) Demonstration.
