Care Advocate Services

The Care Advocate helps older adults and their families understand all the various options for chronic and long-term care. With professional guidance, seniors and their families are able to put together services that best meet individual needs to help maximize independence. The Care Advocate program helps older adults get the right service at the right time in the right place, with an emphasis on appropriate community services, which often can delay, even prevent, nursing home placement.
For each individual, the Care Advocate conducts a thorough assessment, develops a service plan that matches needs and desires, recommends home safety improvements, offers caregiver suggestions and solutions to reduce the risk of at-home accidents, and assists in health care provider and entitlement communications.
